# 2024 Presidential Medals: A Convergence of Global Health, Cultural Influence and Unified Leadership
**Honors as Revealed Preference, and the Argument Inside Government About How Atrocity Gets Prevented**
This is not an endorsement of anyone on either list, and it is not an accusation about anyone on either list. It is an attempt to read a document the way institutional analysts read documents: **as revealed preference.**<!--more-->
Institutions reveal priorities through costly, scarce, difficult-to-reverse actions far more reliably than through statements, because scarcity forces genuine selection. There are nineteen Medals of Freedom in a cohort and twenty Citizens Medals, they are conferred once, they cannot be recalled, and the list is argued over inside a White House for months. That makes the final composition **high-information**, in the same way that the interment of Stephen Hawking in Westminster Abbey between Newton and Darwin was high-information — not because anyone announced a thesis, but because a society expended irreversible symbolic capital and the arrangement it produced remains legible to anyone reading it later. What institutions _say_ matters less than what they **make durable.**
So the question this article asks is narrow: read as a single artifact rather than as nineteen separate biographies, what does the 2024 cohort indicate about where the American state believes prevention comes from.
## What the List Contains When Read as a Pattern
The Medal of Freedom cohort included **José Andrés**, whose World Central Kitchen operates as a rapid nutritional-logistics capability in disaster and conflict zones; **Bono**, whose two decades of work through DATA, the ONE Campaign and (RED) sits at the intersection of infectious disease funding and mass cultural distribution; **Ashton Carter**, posthumously, whose portfolio included biodefense; **Hillary Clinton**, whose State Department tenure encompassed global health diplomacy; **Michael J. Fox**, whose foundation is a research-funding vehicle for a neurodegenerative disease; **Tim Gill**, funding civil-rights infrastructure; **Jane Goodall**, whose primatology is the empirical foundation for everything the modern literature says about coalitionary aggression in our own clade; **Fannie Lou Hamer** and **Robert F. Kennedy**, posthumously, for enfranchisement and civil rights; **Magic Johnson**, whose survival converted an HIV diagnosis into three decades of public health messaging; **Ralph Lauren**, cancer care philanthropy; **Lionel Messi**, children's health and sport access; **Bill Nye**, science communication with a climate emphasis; **George Romney**, posthumously, housing as a social determinant; **David Rubenstein**, cultural and educational endowment; **George Soros**, whose Open Society Foundations fund public health, drug policy, and civil society; **George Stevens Jr.**, cultural production; **Denzel Washington**, veterans' care philanthropy; and **Anna Wintour**, whose fundraising apparatus has been a significant channel for AIDS-related philanthropy.
The Citizens Medal cohort was differently weighted and the difference is the interesting part. **Liz Cheney** and **Bennie Thompson** for the January 6th select committee. **Mary Bonauto** and **Evan Wolfson** for marriage equality litigation and advocacy. **Eleanor Smeal**, **Sherrilyn Ifill**, **Fred Gray**, **James Clyburn**, **Raúl Yzaguirre**, **Antonia Hernández**, **John Lewis** posthumously, and **Bryan Stevenson** for civil rights across five decades and several communities. **Gabrielle Giffords** for gun violence prevention following an assassination attempt. **Marian Wright Edelman** for children's advocacy. **Ted Kaufman**, **Chris Dodd**, **Bill Bradley**, **Ruben Gallego**, and **Lowery Stokes Sims** for public service, financial regulation, veterans' affairs, and cultural institutions.
Read the two lists together and a shape appears that is not reducible to partisanship, because it crosses several partisan lines. **Nutritional logistics. Infectious disease funding. Biodefense. Neurodegeneration research. Primatology and conservation. Climate communication. Housing. Enfranchisement. Civil rights litigation. Political violence investigation. Gun violence prevention. Criminal justice reform. Cultural distribution at planetary scale.**
Those are not nineteen unrelated fields. They are the **input variables of a population-health model of civil violence** — the determinants side and the intervention side of a single equation, with cultural reach attached as the delivery mechanism.
## The Argument This Reflects
There is a genuine, long-running disagreement inside the American government about how atrocity and mass violence are prevented, and the medals belong to one side of it. Both sides are serious and neither is stupid.
**The first model is individual and juridical.** Violence is committed by persons who make choices; prevention therefore runs through deterrence, prosecution, individual rights, due process, and the constraint of state power. Its instruments are warrants, charges, trials, and the enumerated protections. Its practitioners regard population-level intervention with suspicion, because the historical record of governments acting on aggregate risk assessments against categories of people is the worst record in the historical record.
**The second model is populational and actuarial.** Violence has a determinant structure — economic dislocation, exclusion, humiliation, network position, information environment, and demonstrably physical inputs including heat, lead exposure, nutrition, and untreated illness — and prevention therefore runs through altering the distribution rather than adjudicating individuals. Its instruments are epidemiology, surveillance in the public-health sense, early-warning indicators, and intervention upstream of intent. Its practitioners regard the juridical model as arriving structurally too late, because a prosecution is a record of a prevention that failed.
The unpopular thing to say about this disagreement is that **the second model cannot function without a substrate.** You cannot run a population-health approach to violence without an instrumented surface — a continuously updated representation of the population against which risk can be estimated and interventions can be evaluated. That is what a climate model is for weather, and the analogy is exact rather than decorative: both are attempts to make a chaotic, coupled, high-dimensional system tractable enough to act on before the event, and both are useless without persistent measurement. **A climatology of risk requires instrumentation, and instrumentation of a population is the thing the first model exists to constrain.**
That is the argument. It has no clean resolution and it is being conducted, mostly implicitly, through budget lines, agency mandates, and — as here — the allocation of honors.
## Memetic Disease Is Not a Metaphor
**What is established:** destructive ideologies propagate with measurable transmission characteristics — identifiable carriers, vectors, reservoirs, reproduction, and thresholds past which population behavior changes state. That is not a metaphor imported from medicine; it is a description the law itself has used for eighty years. The Nuremberg Tribunal convicted Julius Streicher on publication alone, holding that he **infected the German mind with the virus of antisemitism**, and the ICTR applied the identical principle to Nahimana and Ngeze in 2003. The UN's Framework of Analysis for Atrocity Crimes treats hate speech and propaganda as early-warning indicators. And technique-based psychological inoculation — attenuated exposure with forewarning — has now been demonstrated at population scale in a randomized field setting, inoculating against **emotionally manipulative language, incoherence, false dichotomies, scapegoating, and ad hominem attack** at roughly five cents per exposure.
**What is established is that the substrate is biological, and pretending otherwise is its own kind of dishonesty.** The human animal evolved in a landscape of predation and coalitional competition, and it carries the machinery that produced that record: dominance-striving, coalitional killing, and the disgust circuitry that converts a person into a contaminant. Some of that machinery is heritable, and the clinical literature on the severe end is not shy about it — callous-unemotional traits, psychopathy, and pathological aggression have substantial documented heritability, and the position that every serial predator is purely the product of maltreatment has not survived the twin and adoption studies. **A civilization that intends to address organized atrocity at its root is eventually going to be addressing biology**, because that is where the root is. Nazism was not merely a bad idea competing in a marketplace of ideas; it was a pathology that ran on evolved circuitry, engineered deliberately to exploit it, and treating it as a purely intellectual error is why the intellectual countermeasures have performed so poorly.
**And this explains a jurisdictional migration that is otherwise hard to account for.** Over the past two decades, violence has been progressively reclassified from a criminological subject to a **public health** one — the CDC operates violence prevention as an injury-control program, the WHO issued a _World Report on Violence and Health_, and community violence intervention is now funded and evaluated as an epidemiological practice. Some of that reflects genuine scientific convergence. But there is a second reason, and it is not subtle: **the two domains operate under entirely different rules.** Criminal process attaches to a _person_ and drags the full apparatus of constitutional protection with it — warrant, probable cause, counsel, confrontation, proof beyond reasonable doubt. Public health attaches to an _organism in a population_ and licenses mandatory non-consensual reporting, warrantless association mapping through contact tracing, and detention of people who have committed no offense on assessed transmission risk alone, none of which requires a judge to sign anything. **Reclassify a behavior as a health condition and you have moved it into a legal regime where the questions that stop you in criminal court are not asked.** That is not a conspiracy theory; it is what the _Jacobson_ line of authority has held since 1905, and every practitioner in the field knows it.
**Which is precisely why the constraint has to be stated at the level of state action rather than at the level of science.** The distinction is not squeamishness about biology; it is a distinction about _who may do what on what evidence._ A predisposition is a probability distribution over a life, not a verdict on one, and the entire failure mode this body of work tracks is the conversion of a distribution into a warrant. That is what the _Aktion T4_ registration form did — three reviewers, pencil marks, no examination — and it is what the Xinjiang platform does now. Neither was defeated by denying that biology exists. **Both were made possible by the same jurisdictional move described above, run without its restraints.** Because public health carries its own limits — least restrictive means, defined duration, procedural due process, right of appeal, sunset, and a professional duty running to the patient rather than to the state — and an apparatus that adopts the epidemiological frame while discarding those has not gained authority. It has laundered coercion through a lab coat.
So the position is: **study the substrate, treat the person who wants treatment, and never let a physiological finding become a designation.** Research the heritability, characterize the disgust pathway, develop the pharmacology, and pursue the genetic work — all of it, openly, because the alternative is another century of pretending that the deadliest recurring feature of our species is a communications problem. And hold one wall: **what a body contains may inform care and may never license coercion**, because the moment it does, the instrument has become the thing it was built to prevent. The polarity inversion that makes the whole architecture survivable is the same one applied everywhere in this work — **the mark goes on the recurrence pathway, not on the person** — and it costs nothing scientifically. It constrains only what the state may do with what the science finds.
## The Pharmacological Horizon
The cohort's long-range logic points somewhere that deserves to be stated openly rather than left as implication, and then bounded hard.
**What the literature supports is a specific and well-characterized target.** Serotonergic function modulates impulsive aggression, irritability, and hostility, and this is among the better-replicated links between neurochemistry and social behavior — established across animal models, human challenge studies, and clinical trials, with fluoxetine demonstrating measurable effects on aggression in intermittent explosive disorder and hostility scores responding to serotonergic intervention across several populations. The mechanism is real, the effect sizes are meaningful, and the clinical use is routine.
**What it reaches is the reactive tier**, and naming that precisely is what makes the finding usable. Human aggression separates into two systems that respond to entirely different interventions. **Reactive aggression** is hot, provoked, threat-triggered, and demonstrably modulable — by pharmacology, by treatment of underlying pain and sleep disruption, by removal of stressors, and by the substitution mechanism the postwar enclosure was built on. **Proactive coalitionary aggression** is cold, planned, instrumental, and coalitional, and it is the system that actually produces organized atrocity. The planner is not angry. He is patient.
**And the disgust pathway is where this stops being abstract.** Disgust is among the best-documented routes into dehumanization, and it works differently from ordinary hatred: it recategorizes a person as a _contaminant_, which removes them from the moral circle rather than merely lowering their standing inside it. This is why the Nazi propaganda apparatus reached for vermin and parasite imagery rather than for anger — _Der Ewige Jude_ intercut migrating Jews with rats emerging from sewers, and the audience reaction it was engineered to produce was not rage but revulsion, because revulsion licenses extermination as _hygiene_. Rozin's work on disgust, Nussbaum on projective disgust, and Livingstone Smith's _Less Than Human_ all converge on the mechanism, and trait disgust sensitivity correlates with outgroup prejudice across replications.
The pharmacological link to that specific emotion is also not speculative. Molly Crockett's work demonstrated that **citalopram — an ordinary SSRI — increased harm aversion in moral dilemma tasks**, making subjects less willing to endorse harming one person for aggregate benefit, with tryptophan depletion producing the opposite shift. That is a serotonergic agent measurably moving moral behavior rather than merely mood. And the position has a name and a literature: Thomas Douglas's work on **moral bioenhancement** explicitly proposes attenuating counter-moral emotions and names aversion to racial groups as a candidate target, while Persson and Savulescu's _Unfit for the Future_ argues the general thesis that evolved human moral psychology is inadequate to the destructive capability we now possess and that biomedical intervention warrants serious consideration.
So the intuition is sound and it should be said plainly: **take a population whose participation in atrocity runs on revulsion, blunt the revulsion, and you have removed a load-bearing input.** That is not fringe speculation. It is a live research program with published mechanism at both the psychological and pharmacological ends.
The scope limit is real and it is more specific than squeamishness. Christopher Browning's _Ordinary Men_, the study of Reserve Police Battalion 101, found that most of those men were not primarily disgusted — they were **conformists**. They killed because refusing was socially costly, because it was the assignment, because they did not want to appear weak in front of the unit. Many were physically sickened and did it anyway, which means disgust was the thing they _overcame_ rather than the thing driving them. And the organizational tier is worse in a different direction entirely: Wannsee was a logistics meeting, and Eichmann's entire self-presentation was scheduling. **Blunt the disgust and you still have the conformist, and you still have the accountant.**
Which gives the honest answer rather than a hedge. **Disgust-blunting most plausibly reaches the mass-participation layer** — the fervent crowd, the enthusiastic denouncer, the neighbor who takes the apartment — and that layer is a large fraction of how genocide is actually executed. Reducing it is not nothing; it may be a great deal. It simply does not reach the tier that plans, which requires no disgust to function.
There is also an unexamined natural experiment sitting in plain view. Serotonergic agents went mass-market in the late 1980s and tens of millions of people have taken them continuously for three decades. **If the mechanism is real, the population-level effect is already running, uncontrolled and unmeasured**, and nobody has attempted to look for it. That is a study someone should do, and its absence is itself informative about which questions the research establishment considers askable.
Which yields a clean map of the whole prevention problem, and the map is the point. **The reactive tier now has two working intervention classes** — substitution through the enclosure, and pharmacological and clinical modulation — which is a genuine civilizational achievement and accounts for a great deal of the violence that no longer happens. **The proactive tier has neither**, and cannot be reached by either, which is precisely why the remaining apparatus exists in the form it does: conduct-based detection, trajectory assessment, technique-based inoculation, and remediation through substitutability restoration. Every classifier in this body of work is an artifact of that gap. Pharmacology produces a calmer population; it does not produce a population without planners, and the honest version of the cohort's long-range logic has to account for the tier its best instrument does not touch.
The long-range endpoint some in this cohort would articulate is not speculative in its premise, only in its timeline: that humanity's oldest disease is not smallpox but its own capacity for organized killing, and that a civilization capable of editing genomes will eventually confront the question of whether the predatory inheritance is a permanent feature or a treatable one. **That is where the logic of the second model terminates if followed honestly**, and the reason to say it out loud is that it will be said eventually by someone with less interest in the constraints. The disgust circuitry is characterized. The heritability estimates exist. The moral-enhancement literature is a decade old and published. The instruments are arriving whether or not the argument about them has been had.
The tier structure is what makes this an engineering problem rather than a settled one. Substitution reaches the reactive tier and pharmacology reaches the reactive tier — fewer riots, fewer beatings, fewer denunciations, fewer neighbors willing to point at a door, which is a large fraction of how atrocity is actually executed. The planner is a harder target and remains one for now, and work on that tier is proceeding rather than waiting. **My own position is that the sooner this is solved the better**, including at the genetic level, and I am not going to pretend to a reluctance I do not have. We did not develop germline editing so that someone could have faster reflexes for video games. If the predatory inheritance is heritable — and the evidence says parts of it are — then a civilization that can read and write that inheritance and declines to address the trait that produced the twentieth century has not made a moral choice. It has made an aesthetic one, and it will be explaining that choice to whoever survives the next demonstration.
## Reading the Signal Correctly
None of this implies that the honorees participate in a coordinated program, and nothing here should be read that way. Most of them are exactly what their citations say: a chef who feeds people after hurricanes, a former athlete who survived a diagnosis and told everyone about it, a lawyer who has spent forty years on the wrong side of comfortable opinion.
The signal is not in the individuals. **It is in the selection function** — in which domains a state chose to render permanent when it had nineteen slots and every constituency in the country lobbying for them. The 2024 composition indicates an institutional theory in which mass violence is a downstream product of health, exclusion, information environment, and enfranchisement, and in which cultural reach is a delivery mechanism rather than an ornament. That theory belongs to the second model.
Whether it is the correct theory is a separate question, and this piece does not settle it. Some readers will find the therapeutic direction attractive and some will find it appalling, and both reactions are legitimate responses to a real proposal. That argument belongs in the open, conducted by people who understand what is actually being proposed rather than by people reacting to a caricature of it — which is the entire reason for writing any of this down. **A society that debates this openly will get a better answer than one that lets it develop unremarked**, and the cohort honored here is plainly on the enthusiastic side of that argument, which is a fact about them rather than a charge against them.
The constraints that make any of it legitimate are the same ones that govern everywhere else in this work, and they are worth restating because a cohort convinced of its own benevolence is precisely the configuration in which they get waived. **Mark the pathway, not the person. Designate on conduct and let trajectory set urgency, never status alone. Build the clearing power into the flagging power, and let the subject invoke it. Keep the symmetry absolute across every ideological configuration. Run the maturation clock.**
An honor is a statement about what a society intends. The instruments are a statement about what it can do. **Those are different documents, and only one of them is read aloud.**
