## 1. Executive posture
The register's architecture holds. Five hundred inferences remain correctly homed, and nothing in the 2026-09-11 to 2026-10-23 record contradicts any of them. What changed is the map of *where* the first approvals arrived, and it changed in a direction the register anticipated without naming.
The first market approval for an implanted brain-computer interface came from China's National Medical Products Administration on 13 March 2026 (Neuracle's NEO, cervical spinal-cord injury), and the first CE mark for a commercially launched BCI retinal implant came from Europe on 22 July 2026 (Science Corporation's PRIMA, geographic atrophy). Both precede any FDA premarket approval. INF-0055 described a race for the first PMA as the race for the classification template; the template contest is now multi-jurisdictional, and the vault holds neither event. That is the single largest gap this pass found.
Three further currents run through the window:
1. **Speech and inner speech are now product-grade research capabilities across three companies.** Paradromics reported real-time attempted *and imagined* speech from its first Connect-One participant (14 September 2026); Neuralink's VOICE trial showed thought-to-speech in a third participant with a synthesized voice (September 2026); and the Stanford–Emory BrainGate team decoded inner speech behind a thought-password gate (*Cell*, 14 August 2025). Cluster D's privacy and access-control inferences now have working instances.
2. **Capital arrived on the schedule INF-0025 predicted.** Five private rounds between June 2025 and September 2026 put roughly $1.58 billion into five U.S. BCI companies (Neuralink $650 million, Synchron $200 million, Science $230 million, Merge Labs $252 million, Precision $250 million).
3. **Embodied connectome emulation reached the fly.** FlyGM (Tsinghua, March 2026) and Eon Systems (March 2026) drove a simulated fly body from the whole-brain connectome; Eon names the mouse as its next target. INF-0126 and INF-0131 move from demonstration toward pipeline.
### 1.1 Corrections to the first draft of this report
The first draft of this file, written earlier on 23 October 2026, contained claims that a verification pass could not sustain. They are corrected here so the next cycle builds on clean ground.
| First-draft claim | Verified record | Effect |
|---|---|---|
| The 850 unresolved labels are "largely built out" | 19 of 850 now resolve to a wiki file (2.2%); the named attractor nodes it listed were never in the 850-label ledger | Ledger remains open; see the companion audit |
| Connexus, Endovascular BCI, Closed-Loop Neuromodulation and the UNESCO Recommendation are missing nodes | All exist: [[wiki/Paradromics Connexus\|Paradromics Connexus]], [[wiki/Connexus BCI\|Connexus BCI]], [[wiki/Endovascular Brain-Computer Interface\|Endovascular Brain-Computer Interface]], [[wiki/Closed-Loop Neuromodulation\|Closed-Loop Neuromodulation]], [[wiki/UNESCO Recommendation on the Ethics of Neurotechnology\|UNESCO Recommendation on the Ethics of Neurotechnology]] | Enrich; create none |
| Medtronic "exists only as a corporate shell" | No Medtronic file exists in the wiki | Creation candidate |
| Synchron completed enrollment in the COMMAND *pivotal* trial in September 2026 | COMMAND is the six-participant early feasibility study; enrollment closed in 2023 (ClinicalTrials.gov NCT05035823, "Active, not recruiting," updated 8 September 2026). Synchron plans to file a pivotal IDE before the end of 2026 | INF-0055 timeline set by the pivotal filing |
| The 312-neuroscientist survey appeared in *PLOS One* in August 2026 | Published 24 June 2025 (DOI 10.1371/journal.pone.0326920) | Pre-audit anchor; still absent from [[wiki/State Sufficiency Problem\|State Sufficiency Problem]] |
| BrainGate2 home use, the double neural bypass, Apple BCI HID, the Convey letter, Merge–Butterfly, and Science's Series C are post-audit events | All entered the vault before 11 September 2026, through the article's September revision | Confirmation, not new evidence |
| Apple is INF-0079; Synchron's nineteen-year arc is INF-0077; "DBS substitution" is INF-0075 | Apple BCI HID is INF-0076 and INF-0077; the nineteen-year arc is INF-0053 (with INF-0475); INF-0075 is rehabilitation and neuroplasticity; INF-0079 is shared autonomy | IDs corrected throughout |
| INF-0001, INF-0490 and INF-0492 should be promoted to strongly indicated | All three already carry that tier | Recorded as confirmations |
| INF-0476, INF-0488, INF-0493, INF-0497 and INF-0498 are "unresolved" | All five are tiered analytic | Corrected |
| Med-X BCI-2035 is the first peer-reviewed DALY-ranked BCI roadmap | It is a DALY-weighted priority ranking by Ning Jiang, Jiayuan He and Xiaohong Sui (9 October 2026); priority of first publication is unverified | Proposed INF-0501 rewritten |
| LNP stiffness tuning (proposed INF-0505) is strongly indicated | One in-vitro cancer cell-line study | Retiered to plausible |

## 2. Top most likely inferences
Ranked by consequence if the inference resolves affirmatively within 24 months. Tier changes are recommendations to the register; the register itself is unchanged.
### 2.1 Recommended tier changes
| INF | Current tier | Recommended | Deciding evidence (dated) |
|---|---|---|---|
| **INF-0465** China runs a parallel program with its own trials, companies and standards | strongly indicated | **established** | Seven-ministry *Implementation Opinions on Promoting Innovation and Development of the BCI Industry* (30 July 2025) with 2027 and 2030 targets; NMPA market approval of Neuracle NEO (13 March 2026); 79.1% non-invasive share across registered Chinese BCI studies (arXiv 2607.07185, July 2026) |
| **INF-0083** inner speech is the first genuinely private content to become instrumentable | strongly indicated | **established** | Willett, Kunz, Meschede-Krasa et al., *Cell*, 14 August 2025: inner speech decoded in four participants, up to 74% accuracy; Paradromics imagined-speech patterns (14 September 2026) |

| **INF-0096** restored speech uses a model-synthesized voice, making the identity object part biological, part trained artifact | strongly indicated | **established** | Neuralink VOICE participant "Terry" speaking through a synthesized voice reported as Grok-based (September 2026); UC Davis restored-voice work in the BrainGate2 lineage |
| **INF-0279** the MIND Act is a study mandate stalled in committee | established | established (reconfirmed) | S.2925 introduced 29 September 2025; no committee action on Congress.gov as of this scan |
### 2.2 Confirmed at current tier, with new anchors
| INF | Tier | New anchor |
|---|---|---|
| **INF-0001** Connexus as a networking device; capability resolves to channels × uptime × egress | strongly indicated | Real-time conversational speech over a wireless link and a live phone call with family (14 September 2026), after the 26 August 2026 FDA letter adding phones, tablets and laptops to Connect-One |
| **INF-0025** capital arrives quickly once the value ratio is this wide | analytic | About $1.58 billion across five rounds, June 2025 to September 2026 (Section 4.1) |
| **INF-0055** first approval sets the classification template | strongly indicated | Template contest is now three-jurisdictional: NMPA (13 March 2026), EU CE (22 July 2026), FDA pending Synchron's planned pivotal IDE |
| **INF-0367** programs migrate toward faster jurisdictions, and trial geography shows where a capability becomes ordinary | strongly indicated | Both first commercial authorizations landed outside the United States |
| **INF-0491** the medical indication is the carrier wave | analytic | Every 2026 authorization or IDE rides one: spinal-cord injury (Neuracle), geographic atrophy (PRIMA), ALS and motor neuron disease (Connect-One, VOICE), treatment-resistant depression (Motif RESONATE IDE, April 2026), stroke (CorTec, second Breakthrough designation, 30 August 2026) |

### 2.3 Held at current tier, with the deciding record named
- **INF-0477** (first chronic implant above 10,000 channels): **plausible, held.** The first draft called it a scheduling question; the verified record holds Connexus at 421 channels and no published 10,000-channel chronic device. Deciding record: a peer-reviewed or regulator-filed channel count from Neuralink, Paradromics, or a Chinese high-channel program.
- **INF-0479** (mouse-region simulation predicting held-out responses): **plausible, held.** The MICrONS foundation model (*Nature*, April 2025) predicts held-out responses from function; the marker requires a connectome-*derived* model. Eon Systems' stated mouse program is the nearest claimant.
- **INF-0017** (cortical visual prosthesis as write-side experiment): **plausible, held.** PRIMA's CE mark authorizes a *retinal* write device; Neuralink's Blindsight had no reported human implant as of 7 October 2026.
- **INF-0480** (wetware deployment above 1,000 units): **plausible, held.** A 120-unit Melbourne installation appears in one secondary source only.
- **INF-0117** (synaptic strength from morphology): **unresolved, held.** The June 2025 survey gives the field's prior (70.5% favor connectivity and synaptic strength; 45.6% judge memory readout from a static snapshot possible in principle; median 41.0% that memories are decodable from an aldehyde-stabilized cryopreserved brain; median human emulation estimate 2125); paired physiology-and-ultrastructure data remain the settling record.
- **INF-0100** (portability of the personal residual is the decade's standards fight, unclaimed): **strongly indicated, held.** No claimant appeared. Apple's BCI HID (May 2025) holds the input layer; ISO/IEC JTC 1/SC 43 holds the reference architecture ([[wiki/ISO IEC 27572 2026\|ISO/IEC 27572:2026]]); the person-state layer remains open.
### 2.4 Near-term resolution watch (next 6–12 months)
1. Synchron pivotal IDE filing (company target: before end of 2026) → INF-0055.
2. First FDA Humanitarian Device Exemption decision for PRIMA → INF-0055, INF-0017.
3. Neuralink peer-reviewed VOICE metrics (latency, vocabulary, accuracy) → INF-0007, INF-0080.
4. A second NMPA invasive-BCI registration (NeuroXess is in a year-long trial) → INF-0465, INF-0352.
5. Eon Systems or FlyGM follow-on to mouse tissue → INF-0479.

## 3. Reasonable inferences proposed for the register (INF-0501 to INF-0512)
Written in the register's grammar so they can be inserted without rewriting. INF-0501 to INF-0505 revise the first draft's proposals; INF-0506 to INF-0512 are new.
**INF-0501** · *Insert:* `wiki/Med-X BCI-2035 Perspective`, [[wiki/Med-X|Med-X]], [[wiki/Brain-Computer Interfaces|Brain-Computer Interfaces]] · *Tier:* analytic — Ning Jiang, Jiayuan He and Xiaohong Sui ranked twelve neurological conditions by urgency, importance and impact using disability-adjusted life years and placed Alzheimer's disease, stroke, depression and anxiety at the top (*Med-X*, 9 October 2026). A DALY ranking is a reimbursement argument written in epidemiological units, and the four named indications are the carriers most likely to fund the next decade of BCI capability.
**INF-0502** · *Insert:* `wiki/Vagus Nerve Stimulation`, [[wiki/Networked Body|Networked Body]] · *Tier:* plausible — Wearable auricular and cervical vagus stimulation with AI-driven closed-loop personalization moves adaptation from hardware to model (*Med-X* review, 25 August 2026). Implanted VNS already carries a U.S. depression indication; a wearable clearance would give Cluster M its first non-invasive prescription node. No 2026 FDA clearance for a wearable depression device was located.
**INF-0503** · *Insert:* `wiki/Tissue Nanotransfection`, [[wiki/Real-Time Genetic Repair|Real-Time Genetic Repair]], [[wiki/In-Body Regulation and Autonomous Physiology|In-Body Regulation and Autonomous Physiology]] · *Tier:* plausible — Tissue nanotransfection turns accessible skin into a biofactory that releases extracellular vesicles acting beyond the application site (*Med-X* Perspective, 1 June 2026). It is the most surgery-free candidate for INF-0325's body-as-platform architecture; a registered first-in-human study is the deciding record.
**INF-0504** · *Insert:* [[wiki/Continuity Evidence Ladder|Continuity Evidence Ladder]], [[wiki/Neurotechnology Ecosystem|Neurotechnology Ecosystem]] · *Tier:* analytic — Across 1,327 correlation analyses in 310 biomedical articles, 1.94% reported confidence intervals and 58.71% omitted sample size (*Med-X*, 22 October 2026). The register's biomedical anchors therefore carry a measurable reporting-quality discount, and anchors with sample size and interval reported deserve heavier weight.
**INF-0505** · *Insert:* `wiki/Lipid Nanoparticle`, [[wiki/Real-Time Genetic Repair|Real-Time Genetic Repair]] · *Tier:* plausible — Low-to-intermediate-stiffness lipid nanoparticles improved mRNA delivery in liver, ovarian and lung cancer cell lines through clathrin- and lipid-raft-mediated uptake (*Med-X*, 1 May 2026). If stiffness tunes tropism in vivo, the redosable delivery INF-0334 requires becomes a materials-property search rather than a chemistry search.
**INF-0506** · *Insert:* `wiki/Neuracle`, `wiki/NMPA`, [[wiki/Neurotechnology Ecosystem|Neurotechnology Ecosystem]] · *Tier:* established anchor, strongly indicated inference — The first market approval for an implanted BCI (NMPA, Neuracle NEO, 13 March 2026) and the first commercial CE-marked BCI retinal implant (PRIMA, 22 July 2026) both preceded any FDA premarket approval. The classification template INF-0055 describes is being written in Beijing and Brussels first, and U.S. sponsors will file against precedents set abroad.
**INF-0507** · *Insert:* `wiki/Inner Speech Decoding`, [[wiki/Semantic Neural Decoding|Semantic Neural Decoding]], [[wiki/Neurorights|Neurorights]] · *Tier:* established — A silently imagined password gated an inner-speech decoder with over 98% recognition, and the same system decoded unprompted counting during a visual task (*Cell*, 14 August 2025). Cognitive access control now exists as an engineering primitive, and its first deployment came from the research team rather than from statute.
**INF-0508** · *Insert:* [[wiki/Capital Routing Node|Capital Routing Node]], [[wiki/Neurotechnology Ecosystem|Neurotechnology Ecosystem]] · *Tier:* analytic — Roughly $1.58 billion entered five U.S. BCI companies in fifteen months, with repeated investors across them (Khosla Ventures in Science and Synchron; Lightspeed in Science and Neuralink). Investor overlap at this density is the integration plan INF-0451 describes, readable before any partnership is announced.
**INF-0509** · *Insert:* [[wiki/Whole-Brain Emulation|Whole-Brain Emulation]], `wiki/Eon Systems`, [[wiki/FlyWire|FlyWire]] · *Tier:* plausible — Whole-connectome fly controllers driving simulated bodies (FlyGM; Eon Systems, March 2026) convert the species ladder of INF-0141 into a software pipeline: connectome, connectome-constrained dynamics, neuromechanical body. The next rung is a mouse cortical region, and the first group to publish it will hold the INF-0479 marker.
**INF-0510** · *Insert:* `wiki/China BCI Industry Implementation Opinions`, [[wiki/China Brain Project|China Brain Project]] · *Tier:* strongly indicated — The national plan targets two or three leading companies by 2030, and secondary reports add provincial quotas (Sichuan, 3,000 invasive BCI operations a year by 2030; Beijing, three to five BCI unicorns by 2027; Shanghai, widespread adoption by 2030) that express installed base as plan quotas, pending primary-text confirmation. Under INF-0492, a planned installed base becomes irreversible on the plan's own schedule.
**INF-0511** · *Insert:* [[wiki/Neurorights|Neurorights]], [[wiki/UNESCO Recommendation on the Ethics of Neurotechnology|UNESCO Recommendation on the Ethics of Neurotechnology]], [[wiki/Continuity Governance|Continuity Governance]] · *Tier:* plausible — The UN Secretary-General's October 2026 report recommends treating thoughts as highly sensitive personal data and requiring explicit consent for neurotechnology in investigative, judicial and correctional settings. Read with Human Rights Council resolution 58/6 (April 2025) and the UNESCO Recommendation (November 2025), it is the third UN instrument in eighteen months; market participants should price a criminal-justice consent rule into product roadmaps for justice-sector buyers.
**INF-0512** · *Insert:* [[wiki/Neuralink|Neuralink]], [[wiki/xAI|xAI]], [[wiki/Cognitive Interface Layer|Cognitive Interface Layer]] · *Tier:* strongly indicated — A Neuralink speech participant speaking through a voice model from a sister company (reported September 2026) is the first visible composition of implant, decoder and foundation-model voice inside one founder's portfolio. INF-0078 and INF-0490 meet here: the realized bandwidth comes from the model's prior, and the model belongs to the same capital graph as the implant.

These are the agglomeration sites INF-0463 predicts; trial-site co-location is the readable signal.
- **[[wiki/Tsinghua University|Tsinghua University]]** sits behind both Neuracle's NEO and FlyGM, joining China's first commercial invasive BCI and a leading embodied-connectome result in one institution.
- **[[wiki/Sam Altman|Sam Altman]]** spans Merge Labs (interfaces), OpenAI (foundation models and Merge's largest investor) and, in earlier reporting, Nectome's waitlist (preservation). Read under INF-0451, the portfolio is a continuity stack in miniature.
- **Founders Fund** appears in Neuralink's Series E and led Until Labs' $58 million Series A (September 2025), which spans interfaces and organ-scale cryopreservation in one investor.

| `Casey Harrell`, `David Brandman` | Named participant and implanting surgeon of the BrainGate2 home-use record |
### 5.3 Missing register targets with the heaviest inference load
The register's own `Insert` fields name these targets; they remain unresolved and are worth creating as canonical concept homes: `Medical Indication as Carrier` (INF-0491), `Installed Base Effect` (INF-0492), `Channel Count Marker` (INF-0477), `Premarket Approval` (INF-0055), `Percept Neurostimulator` (INF-0061), `BrainSense Adaptive DBS` (INF-0062), `Sonogenetics` (INF-0026, INF-0027), `Vagus Nerve Stimulation` (INF-0305), `Lipid Nanoparticle` (INF-0334). The full 831-label residue is in the companion audit.

## 9. Addendum, later on 23 October 2026
### 9.1 Priority 1 nodes created
The ten Priority 1 rows of section 5.1 were created as eleven files (the Medtronic row split into a company node and a device node that resolves the register target `BrainSense Adaptive DBS`, INF-0062): [[wiki/Neuracle|Neuracle]] · [[wiki/National Medical Products Administration|National Medical Products Administration]] · [[wiki/China BCI Industry Implementation Opinions|China BCI Industry Implementation Opinions]] · [[wiki/PRIMA Retinal Implant|PRIMA Retinal Implant]] · [[wiki/Medtronic|Medtronic]] · [[wiki/BrainSense Adaptive DBS|BrainSense Adaptive DBS]] · [[wiki/Inner Speech Decoding|Inner Speech Decoding]] · [[wiki/Neuralink VOICE Trial|Neuralink VOICE Trial]] · [[wiki/Med-X BCI-2035 Perspective|Med-X BCI-2035 Perspective]] · [[wiki/What Are Memories Made Of (2025 Survey)|What Are Memories Made Of (2025 Survey)]] · [[wiki/Vagus Nerve Stimulation|Vagus Nerve Stimulation]]. Each carries a `## Relationships` section, dated sources, and register statements quoted verbatim with their tiers. Verification during creation refined three points: the national plan's 2030 target is two or three leading companies (provincial quotas are secondary-source); NEO is a Class III approval after 36 procedures across 11 hospitals, co-developed with Tsinghua University (Hong Bo's group); and Neuralink credits a Grok Voice model with the VOICE trial's spoken output (post of 18 September 2026).
### 9.2 Bearing of the new Med-X entries
[[wiki/Med-X|Med-X]] gained Volume 1 to 3 entries during this pass. Fifteen of them anchor register inferences; the strongest are listed first.

The additions widen Med-X's routes beyond BCI into Cluster M (closed-loop physiology) and Cluster N (delivery), which strengthens the typed-edge proposals in section 6.2. The node itself was mid-edit at the time of reading: its Volume 3 list was written with literal `\n` sequences and a stray `TESTLINE1 TESTLINE2` line, and a `Med-X-BROKEN-BACKUP.md` file sits beside it. Later the same day, on request, [[wiki/Med-X|Med-X]] was rebuilt from that backup with all 93 papers as individual entries threaded to wiki nodes and inferences, and the backup file was removed.

The requested `wiki/Interfaces and Continuity Architecture` does not exist and was treated as the Neural Interfaces router. The 850-label ledger was re-resolved by exact and case-insensitive filename match plus frontmatter aliases. Every factual claim in the first draft was checked against a primary or near-primary web source; secondary-only claims are marked as such. Tier labels refer to the inference, never to every anchor inside it.
