# AI Incident Reporting
**Entity class:** Governance mechanism
**Domain:** Artificial intelligence / safety / accountability
**Maturity:** Developed
## Definition
AI incident reporting is the structured disclosure of failures, near misses, misuse, loss of control, and consequential harms involving AI systems.
## Mechanism and significance
Reporting creates a shared evidence base for identifying recurring failure modes, assigning responsibility, improving controls, and coordinating responses across laboratories, deployers, regulators, and states.

## Evidence boundary
A reporting channel does not itself prevent incidents. Its value depends on scope, incentives, protection against concealment, technical comparability, and the ability to translate reports into accountable action.
## Simple Reminders, Quotations, and Thoughts
> A classic twentieth-century task force is structurally mismatched to a distributed twenty-first-century technology. Administrative centrality is not the same thing as technical control.

> Distributed AI needs scaffolding rather than imagined central control: incident reporting, identity, liability, bounded experiments, instrumentation, published failures, and accountable responses. Governance should reduce the blast radius without pretending it can centrally command an intelligence explosion.

> A bilateral AI hotline may help two powers manage agents running amok without ending their competition. An incident channel can reduce misunderstanding while the underlying superintelligence arms race continues.
