## Definition
**Accountability mechanisms** are procedures and institutions that make an actor answerable for a decision, expose the basis and consequences of that decision, and provide a credible route to correction, remedy or sanction.
Accountability is stronger than visibility alone. [[wiki/Transparency|Transparency]] can reveal conduct, but accountability requires an authorized process capable of changing what follows from that conduct.
## Core Functions
- identify who held authority and what obligations applied;
- preserve evidence about actions, inputs and outcomes;
- permit affected parties to challenge a decision;
- assign responsibility without collapsing complex systems into a single scapegoat;
- correct harmful state across downstream systems;
- apply proportionate remedy, consequence or institutional redesign.
## Operational Forms
Mechanisms may include audits, public reporting, contractual review, legislative or judicial oversight, independent inspection, complaint processes, [[wiki/Appeals Mechanisms|appeals]], service-level enforcement and technical logs that preserve decision provenance.
In automated systems, the mechanism must reach both the responsible institution and the operational representation through which the decision was made. Explanation without authority to correct the relevant state is not an effective remedy.
## Relation to Infrastructure

## Failure Modes
Accountability fails when responsibility is diffuse but remedy is centralized, when review bodies cannot alter outcomes, when evidence is inaccessible, when sanctions substitute for repair, or when an automated rule is treated as neutral merely because it executes consistently.
