It replaces the narrow question—“How many records were wrong?”—with the system question: **How much human and institutional damage did each wrong state produce before and after correction?** The measure includes: - adverse encounters and their duration; - agencies, databases, and jurisdictions reached; - derivative decisions made while the state remained active; - family members, associates, or organizations pulled into records; - detention, search, delay, denial, and mobility loss; - work, education, immigration, health, and relationship effects; - direct expenses, lost income, replacement travel, and legal cost; - psychological load, vigilance, distrust, and behavior change; - time required for adjudication and downstream synchronization; - repeated encounters after nominal correction; and - residual records or consequences that survive repair of the source state. One form error generated physical detention, travel interruption, student-visa consequences, inability to return to Stanford, international watchlist effects, harm to her daughter's travel, and nearly a decade of litigation.
