## Definition
**Neuracle Technology (Shanghai) Co., Ltd. is the Chinese company whose NEO implant received the first market approval for an implanted brain-computer interface.** China's National Medical Products Administration approved NEO as a Class III medical device on 13 March 2026 for hand-motor function in adults with cervical spinal-cord injury. Xinhua reported it as the world's first market launch of an invasive BCI medical device.
## Device
NEO is a coin-sized, wireless implant that records cortical signals through electrodes placed in the epidural space: under the skull, above the dura, and off the brain tissue itself. Decoded grasp intent drives a pneumatic glove, letting the user grasp objects and drink unaided. The single implantation is designed for long-term use, and patients operate the system at home about one month after surgery. Coverage differs on whether to call the placement invasive or semi-invasive; the approval treats it as an implanted Class III device. Its channel count has not been published in a primary source.
The device was developed with Tsinghua University, where Hong Bo leads the academic group. The first human implantation took place at Xuanwu Hospital in Beijing in October 2023, in a participant paralyzed for fourteen years by a complete cervical injury; Tsinghua reports grasp accuracy above 90% after rehabilitation.
## Approval record
| Item | Record |
|---|---|
| Regulator | National Medical Products Administration |
| Approval | 13 March 2026, Class III |
| Indication | Adults aged 18 to 60 with quadriplegia from cervical spinal-cord injury (C2–C6) who cannot perform finger grasping |
| Evidence base | 36 procedures: 4 feasibility participants and 32 in a multicenter GCP trial across 11 hospitals |
| Reported outcome | All participants improved grasp function to varying degrees; some showed signs of neural remodeling |
| Reimbursement | An insurance code from the National Healthcare Security Administration within about twelve days of approval (single-source report) |
## Forward Capability
Neuracle holds the first commercial classification template for an implanted BCI, written by a Chinese regulator before any FDA premarket approval. U.S. sponsors such as Synchron and Paradromics will file against a world in which that precedent already exists. The epidural geometry is a commercial choice: lower surgical risk than penetrating arrays, a home-use profile within a month, and an indication with a defined patient population. Shanghai counts 60 innovative BCI enterprises, three of whose invasive products are in the NMPA special review procedure for innovative devices; Neuracle is the first through.
## Open Questions
- The channel count, sampling rate and decoder architecture of NEO, from the registration file or a peer-reviewed report.
- The insurance-code status and patient cost, from the National Healthcare Security Administration.
- Whether NEO's evidence package will be filed outside China, and against which regulator first.
## Research Inferences
These entries translate the forward-looking register in Research Inferences into ordinary wiki prose. The tier labels apply to the inference, not automatically to every factual anchor inside it. Proposed IDs above INF-0500 and tier recommendations come from the 2026-10-23 cycle: Research Inferences Integration Report — 2026-10-23 · Research Inferences Integration Audit — 2026-10-23.
- **INF-0055 — Strongly indicated.** The first PMA for a permanently implanted BCI creates the classification template every subsequent device is measured against. Regulatory precedent is a form of standards authorship, and it is being competed for by a small number of firms on a two-to-three-year horizon.
- **INF-0367 — Strongly indicated.** Clinical programs migrate toward jurisdictions with faster pathways, which relocates the evidence base for frontier interventions. Reading where trials are run is the most reliable indicator of where a capability will first become ordinary.
- **INF-0465 — Strongly indicated** (recommended for promotion to established in the 2026-10-23 report). Substantial neural-interface and brain-mapping programs operate outside the American reporting environment, with their own trials, companies, and standards participation. Visibility in English-language press is not a measure of capability, and native-language sourcing is required for an accurate picture.
- **INF-0491 — Analytic.** Every capability in this corpus enters the world through a medical indication because that is the funded, permitted, and reimbursable route. The indication is the carrier wave, and the capability is the signal.
- **INF-0506 (proposed) — Strongly indicated.** The first market approval for an implanted BCI (NMPA, 13 March 2026) and the first commercial CE-marked BCI retinal implant (PRIMA, 22 July 2026) both preceded any FDA premarket approval; the classification template is being written in Beijing and Brussels first.
