## Definition
**The Plausible Mechanism Framework is the FDA's route for developing individualized therapies against specific genetic conditions with a known biological cause, published as draft guidance on 25 February 2026.** Its full title is *Considerations for the Use of the Plausible Mechanism Framework To Develop Individualized Therapies That Target Specific Genetic Conditions With Known Biological Cause*. It permits first-in-human studies on preclinical proof of concept and allows new targets under a single application. Commissioner Marty Makary and CBER director Vinay Prasad set out the thinking in the *New England Journal of Medicine*, using Baby KJ as the model case.
## What it requires
As reported in coverage of the draft and the journal article, a sponsor must show a specific molecular or cellular abnormality; a therapy that targets the underlying biological origin in a disease with a well-characterized natural history; and confirmation that the target was successfully edited or drugged, from animal models or, in some circumstances, non-animal models. The scope is severely debilitating or life-threatening conditions in very small populations, and it names genome editing and RNA-based therapies while noting the concepts may extend to other individualized therapies. Several law-firm analyses stress that it establishes no new approval pathway: it describes how evidence from focused trials, mechanistic analysis and natural history may be assembled.
## Forward Capability
Regulatory throughput, not editing chemistry, was the binding constraint on individualized therapy, and this framework relaxes it directly. The vehicle that would make it routine is the umbrella protocol: a customizable prime-editing platform covering seven urea cycle disorders was discussed with the agency as an umbrella-of-umbrellas, which converts regulation from per-drug to per-method. The register's marker for n-of-one medicine becoming ordinary is approval of a platform rather than a product.
## Open Questions
- Whether a final guidance issues, and what it changes from the draft.
- The first umbrella application covering multiple patient-specific guides under one protocol.
- How off-target surveillance obligations are set when each dose is its own manufacturing run.
## Research Inferences
These entries translate the forward-looking register in Research Inferences into ordinary wiki prose. The tier labels apply to the inference, not automatically to every factual anchor inside it. Proposed IDs above INF-0500 come from the 2026-10-23 cycle: Research Inferences Integration Report — 2026-10-23 · Research Inferences Integration Audit — 2026-10-23.
- **INF-0328 — Established.** The FDA's February 2026 draft framework permits first-in-human studies for individualized genetic therapies on preclinical proof-of-concept and allows new targets under a single application. Regulatory throughput, not editing chemistry, was the binding constraint, and this guidance directly relaxes it.
- **INF-0329 — Strongly indicated.** An umbrella-of-umbrellas protocol covering seven urea cycle disorders under one platform converts regulation from per-drug to per-method. Approving a method rather than a molecule is the change that makes n-of-one medicine economically possible.
- **INF-0333 — Established.** The field's argument that the FDA pathway converts an economically unworkable model into a scalable clinical reality is being made by the people who built the editing tools. Practitioner advocacy shaping regulatory design is the normal mechanism by which capability becomes permitted.
- **INF-0326 — Established.** An infant with CPS1 deficiency received a bespoke base-editing therapy designed for his individual mutation, dosed in February 2025 — the first personalized in vivo gene-editing treatment in a human. The unit of therapy became one patient, which is a structural break with every prior drug development model.
