## Definition
**Prime editing writes a specified DNA sequence at a chosen site without making a double-strand break.** A reverse transcriptase fused to a targeting protein copies the intended sequence from an attached guide RNA directly into the genome, so the edit is carried as a parameter rather than requiring a redesigned tool for each target. That property is what makes therapy development resemble software work: the molecule changes, the method does not.
## Platform position
Prime editing is the technique behind the register's strongest platform-approval argument. An umbrella-of-umbrellas protocol covering seven urea cycle disorders under a single customizable prime-editing platform was discussed with the FDA under the Plausible Mechanism Framework, and reported in the *American Journal of Human Genetics* in 2026. Approving a method rather than a molecule is the change that makes n-of-one medicine economically possible; the same commentary cautions that an abbreviated path still depends on meeting evidentiary requirements ordinarily reserved for late-stage trials.
## Forward Capability
Where base editing addresses single-letter changes, prime editing addresses insertions, deletions and replacements, which widens the share of pathogenic variants a single platform can reach. Combined with redosable lipid-nanoparticle delivery and designed enzymes, the therapeutic becomes a computationally specified artifact, and a sequence transmitted to a site near the patient could in principle be synthesized there rather than shipped.
## Open Questions
- The first umbrella investigational new drug application covering multiple patient-specific guides under one protocol.
- Editing efficiency in post-mitotic tissue, the condition that matters for neurons.
- Delivery of the larger prime-editing machinery across the blood-brain barrier.
## Research Inferences
These entries translate the forward-looking register in Research Inferences into ordinary wiki prose. The tier labels apply to the inference, not automatically to every factual anchor inside it. - **INF-0330 — Established.** Prime editing writes specified sequences without double-strand breaks, making the edit a parameter rather than a redesign. When the therapeutic is a string, therapy development becomes a software-like activity with software-like iteration speed.
- **INF-0329 — Strongly indicated.** An umbrella-of-umbrellas protocol covering seven urea cycle disorders under one platform converts regulation from per-drug to per-method. Approving a method rather than a molecule is the change that makes n-of-one medicine economically possible.
- **INF-0333 — Established.** The field's argument that the FDA pathway converts an economically unworkable model into a scalable clinical reality is being made by the people who built the editing tools. Practitioner advocacy shaping regulatory design is the normal mechanism by which capability becomes permitted.
- **INF-0346 — Analytic.** When the enzyme is designed, the guide is a string, and delivery is a formulation, the whole therapeutic becomes a computationally specified artifact. The iteration loop for medicine begins to resemble the iteration loop for software, which changes its improvement rate.
- **INF-0347 — Plausible.** A therapy specified as sequence data can be transmitted and synthesized near the patient rather than shipped. Manufacturing at the point of care is the logical endpoint of a fully digital therapeutic specification, and it makes distance irrelevant to treatment.
