James Lee, who reported conversion of skin to vascular tissue in mice and pigs (*Nature Nanotechnology*, 2017). A *Med-X* Perspective of 1 June 2026 reframes the method as a therapeutic platform: the reprogrammed patch becomes a biofactory releasing extracellular vesicles and bioactive molecules that act beyond the application site. ## Why it matters to the corpus The register's body-as-platform reading requires sensors, actuators, a communication layer, a control loop and updatable software. TNT supplies a manufacturing layer for that platform and places it in the most accessible tissue in the body. Where in-body CAR-T generation removes the cell-manufacturing facility from the cost structure of cell therapy, TNT removes the need for surgical access to the target organ: the factory sits in the skin and signals systemically through vesicles. ## Open Questions - A registered first-in-human study; the 2017 team projected clinical trials within a year and no registration has been located. - Dose control: how output is titrated once a patch is releasing vesicles systemically. - Durability of the reprogrammed state and whether the patch can be switched off. ## Research Inferences These entries translate the forward-looking register in Research Inferences into ordinary wiki prose. The tier labels apply to the inference, not automatically to every factual anchor inside it. Proposed IDs above INF-0500 come from the 2026-10-23 cycle: Research Inferences Integration Report — 2026-10-23 · Research Inferences Integration Audit — 2026-10-23. - **INF-0313 — Strongly indicated.** Generating engineered cells inside the patient rather than in a manufacturing facility removes the cost structure that limits cell therapy to a few thousand patients a year. In-body manufacturing is what converts a bespoke procedure into a distributable product. - **INF-0325 — Analytic.** Sensors, actuators, a communication layer, a control loop, and updatable software describe an operating environment, and the body now has all five. Treating physiology as a managed platform is the framing under which the next thirty years of medicine will be specified. - **INF-0307 — Plausible.** If an implanted controller can read and write autonomic traffic, homeostatic setpoints become adjustable parameters rather than fixed biology. Adjustable setpoints are the mechanism by which medicine crosses from treatment into specification of the body's operating conditions. - **INF-0347 — Plausible.** A therapy specified as sequence data can be transmitted and synthesized near the patient rather than shipped. Manufacturing at the point of care is the logical endpoint of a fully digital therapeutic specification, and it makes distance irrelevant to treatment. - **INF-0503 (proposed) — Plausible.** Tissue nanotransfection reframes accessible tissue as a distributable bio-manufacturing layer: engineered cells at the application site release extracellular vesicles and bioactive factors that act systemically. It is the physical carrier candidate for body-as-platform architectures, and it lowers the need for organ-level surgical access. Proposed in the 2026-10-23 cycle. ## Relationships - **published_in:** Med-X (Perspective, 1 June 2026) and *Nature Nanotechnology* (2017). - **cluster_home:** In-Body Regulation and Autonomous Physiology · Real-Time Genetic Repair. - **mechanism_adjacency:** extracellular vesicles, reviewed in Med-X (16 December 2024), are the messenger TNT relies on. - **delivery_peer:** Lipid Nanoparticle (systemic chemistry) against TNT's local, device-applied route. - **regenerative_context:** Regenerative Medicine · Regenerative Systems. - **ecosystem:** Neurotechnology Ecosystem · Neurotech. ## Sources - [Repurposing the skin to heal the body: Tissue Nanotransfection (TNT) as a therapeutic platform — Med-X, 1 June 2026](https://link.springer.com/article/10.1007/s44258-026-00084-8) - [Topical tissue nano-transfection mediates non-viral stroma reprogramming and rescue — Nature Nanotechnology, 2017](https://www.nature.com/articles/s41565-017-0016-8) - [Apoptotic extracellular vesicles: mechanisms, applications, and therapeutic potential — Med-X, 16 December 2024](https://link.springer.com/article/10.1007/s44258-024-00043-1) As of 23 October 2026.
